How to Access Immunization Records
Online
- Visit http://izrecord.nv.gov
- Provide:
- First and Last names (when vaccinations were given)
- Date of birth (birthday)
- Gender (male/female)
- Social Security Number (SSN)
- For questions or problems, call: (775) 684-5954
Offline
HIPAA regulations prohibit us from faxing records to schools and/or parents or even discussing an individual's immunization records over the phone.
In Washoe County
Washoe County residents need to pick up immunization records in person Monday through Friday between 8 am and 4:30pm
Immunization records will not be released to a third party such as a step-parent, aunt, uncle, adult sibling or to a parent of a child/children 18 years and older with out a completed Authorization for Release of Information form. It you are a legal guardian, you need to bring appropriate documentation.
Outside Washoe County
Individuals living outside of Washoe County need to complete an Authorization for Release of Information form and fax it to (775) 328-8109. Please complete all areas highlighted in yellow. Please allow 3-5 working days for processing.
