All forms must be filled out fully and legibly in print, signed by the voter (signatures are required for all requests and changes to a voter's record) and submitted to the Washoe County Registrar of Voters office for processing. When your order has been completed, the department will contact and notify you.
Completed forms can be mailed, faxed, or emailed to our office.
