The Washoe County Health District, Office of Vital Statistics, charges fees in accordance with Nevada Revised Statute and the State of Nevada Office of Vital Statistics. On October 15, 2010, the State of Nevada authorized an increase in fees to go into effect beginning December 1, 2010.

Any mail-in requests postmarked prior to December 1, 2010, will be honored at the current fee. Beginning December 1, 2010, applications will be returned to the applicant if the full amount is not provided which will delay the processing of the request.
